Just to let everyone know, the phones are going in and out at the facility. Please know that the nurses have their cell phones for emergencies and to notify our residents’ loved ones if needed. We have a couple of cell phones for emergency use also. Those numbers are 606-216-4392 and 606-216-4402. Please know your loved ones are being cared for as always. All utilities are up and going at this time.
Hazard Health and Rehabilitation Center
We provide 24-hour nursing care and long and short term rehabilitation services. We also offer physical, occupational, and speech therapy.
